To know our patients better, we need to collect the following information:

  •  Full name
  •  Date of birth
  •  Gender
  •  Contact details
  •  Emergency contact details
  •  Adverse drug reactions
  •  Current health status
  •  Relevant past health history
  •  Immunisations
  •  Relevant family history
  •  Aboriginal and Torres Strait Islander status
  •  Current medicines list including complementary medicament
  •  Health risk factors like smoking, nutrition, alcohol and physical activity
  •  Relevant social history including cultural background where relevant
